Scion Nonprofit Staffing is leading an immediate search for a Clinical Care Coordinator for our client, a nonprofit organization passionate about providing mental health care, social services, and academic support. This is a full-time, permanent position reporting onsite at our client’s San Francisco, CA location.

RESPONSIBILITIES
- Complete assessment, diagnosis, and individualized treatment plans.
- Coordinate and implement overall treatment and services for assigned clients.
- Chart mental health notes, outcome measures, incident reports, and other documentation.
- Aid with case planning and consultation with team members on clinical issues as appropriate.
- Develop Family Support Teams
- Meet with clients and caregivers on a weekly basis to further treatment goals.
- Provide therapy for alternate cases held by other team members.
QUALIFICATIONS
- Master’s degree in a relevant field
- LCSW or LMFT license
- Valid California Driver’s License
- Ability to respond to crisis situations.
