Scion Staffing has been engaged to conduct a search for a Licensed Vocational Nurse (LVN) Care Coordinator. This exciting onsite opportunity is located in Irving, TX, and offers a rewarding role supporting care coordination and patient education in a collaborative healthcare setting.
We are seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) Care Coordinator to support patients through telephonic and virtual outreach. This role focuses on preventative care, follow-up coordination, and collaboration with clinical teams to improve patient outcomes and access to care. The ideal candidate will bring strong clinical judgment, communication skills, and comfort with care planning protocols.
Deliver patient care coordination via phone and virtual platforms using evidence-based guidelines
Review and evaluate medical records to identify care gaps and support follow-up needs
Educate patients on their care plans, chronic disease management, and next steps
Collaborate closely with Primary Care Providers, Nurse Navigators, and interdisciplinary teams
Manage a high-volume caseload with strong attention to accuracy and timely follow-up
Ensure compliance with Medicare practices and organizational protocols
Active LVN License in the state of Texas or a valid compact state license
Any disciplinary history related to licensure must also be disclosed
Experience with Medicare, telephonic or virtual patient engagement, and evidence-based protocols
Proven ability to collaborate with clinical teams and support patient education
Strong communication and time-management skills
Must be eligible to work onsite in Irving, TX
This is a full-time, onsite role in Irving, TX, offering a competitive pay rate of $30 per hour. Benefits eligibility may vary depending on assignment length and structure.
