Scion Nonprofit Staffing has been engaged to conduct a search for a Case Manager for immediate hire for our client, a wonderful Nonprofit organization that aids the underserved population around the country! This is an exciting Full-Time, Direct-Hire opportunity located in, Los Angeles, CA. The schedule is Sunday - Thursday or Tuesday - Saturday. 

As the Case Manager, you will evaluate and support patients by addressing housing, employment, education, financial, and health needs, facilitating intake processes, coordinating care with partners, providing guidance to staff, conducting crisis intervention, maintaining confidential files, and overseeing various program-related tasks to ensure the well-being and successful integration of clients into the community!

RESPONSIBILITIES:
  • Evaluate patients referred to the program, identifying issues, needs, or obstacles in areas such as Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric.
  • Collaborate with personnel and community partners to handle eligibility documentation and arrange mental and physical health services for clients.
  • Lead the client intake process and organize client files, ensuring completeness of all program eligibility documentation.
  • Work with clients to develop Individual Action Plans.
  • Offer guidance to staff on client action plans, intervention strategies, progress notes, and discharge planning during weekly program meetings and as needed.
  • Provide crisis intervention and support staff with client issues.
  • Manage confidential client files professionally, ensuring compliance with organization standards and the most recent forms.
  • Meet with clients as specified by program requirements and document progress notes in Data/Assessment/Plan format.
  • Conduct home visits for case management services and to ensure the health and welfare of clients.
  • Facilitate client life skills classes.
  • Provide specialized case management services and life skills groups/classes to specific subpopulations.
  • Conduct random drug and alcohol screenings of clients.
  • Uphold confidentiality in compliance with HIPAA standards.
  • Supervise student interns, including task assignment, reviewing process recordings, supervision sessions, evaluations, and adherence to university standards.
  • Complete accurate and timely data entry into the HMIS system and other required internal and external databases.
QUALIFICATIONS:
  • 2+ years of Case Manager experience
  • Experience working with veterans and the homeless population
  • Master’s Degree in Social Work, Psychology, Counseling, or a related field is required
  • Certification in Substance Abuse Counseling is a plus
  • Strong communication and interpersonal skills

COMPENSATION AND BENEFITS:
This exciting career opportunity allows you to work with a brilliant and thriving team! The compensation for this role is $64,000 - $69,000 annually, plus a comprehensive benefits package that includes Medical Insurance, Dental Insurance, a 401K Plan, a Flexible Spending Account, Paid Holidays, Paid Time Off, and Sick Leave.
