Scion Staffing has been engaged to conduct an immediate search for an experienced Community Health Worker for our client, an established nonprofit in the leadership and workforce development space in Los Angeles, CA!

What you will be doing:
-
Visit day laborer sites regularly and other potential target areas with the goal of providing health education and share direct services resources

Identify priorities for referrals and education of the target populations. These include but are not limited to: chronic disease management, mental health, and occupational health.

Prepare materials for outreach efforts, including printed educational materials and sign in sheets for all Health Programming.

Work closely with the Health Team to prepare for monthly Health and Safety Committee meetings with workers. This includes preparing agendas and materials to ensure goals of the Committees are met.

Work strategically with the Health Team to ensure recruitment and retention of workers in Health Programming.

Support the development of the Health and Safety Committees. This includes facilitating health and safety trainings, taking meeting notes, documenting highlights of meetings by taking photos or quotes of participants, and following up on worker issues raised during meetings.

Provide follow up by scheduling one on one visits, calls or texts with workers that need “warm referrals” to health services; and following up to ensure members are being supported throughout the process and receiving necessary services, as well as troubleshooting with agencies if problems arise.
What you will need:

1+ years of experience in health education and referrals

An ability to work onsite in Los Angeles

Bilingual in Spanish
