Scion Staffing has been engaged to conduct a search for a Nonprofit Social Worker for an established medical nonprofit in San Jose, CA! This is a full-time, onsite, direct-hire opportunity!
The ideal person for this role will have a strong background in program management and leadership, preferably in the nonprofit space or supporting individuals with disabilities. They will possess excellent communication, organizational, and problem-solving skills, along with the ability to manage budgets, allocate resources effectively, and build collaborative partnerships with community organizations.

In this critical role, you will guide clients through their vision loss journey, provide personalized care plans, facilitate support groups, and connect them with essential resources to regain independence and confidence.

RESPONSIBILITIES:

Conduct comprehensive assessments to understand clients’ needs, strengths, and goals

Develop and implement personalized care plans that promote independence and well-being

Provide one-on-one and group counseling, addressing emotional and practical challenges

Connect clients with essential community resources, including transportation and financial assistance

Collaborate with multidisciplinary team members to ensure holistic support for clients

Maintain accurate and confidential case records
QUALIFICATIONS:

Bachelor’s degree in Social Work or Marriage & Family Therapy (Master’s degree preferred)

Experience working with older adults, individuals with disabilities, or those facing complex medical conditions

Strong communication and counseling skills with a client-centered approach

Demonstrated empathy, problem-solving mindset, and social perceptiveness

Ability to work independently and travel to multiple locations within San Mateo County

Bilingual candidates are a plus!
COMPENSATION AND BENEFITS:
This position offers an annual salary range of $65,000 - $85,000, along with a comprehensive benefits package including health, dental, vision, and life insurance. Employees also enjoy paid time off and a supportive, mission-driven work environment that values professional growth.
